Disrupted Muscle Clocks and Poor Diet May Drive Diabetes, Study Finds

Disruptions to the body's internal clock—specifically in muscle tissue—combined with an unhealthy diet may significantly increase the risk of developing diabetes, according to new research from Northwestern Medicine. The study, published in Proceedings of the National Academy of Sciences, sheds light on the critical role circadian rhythms play in muscle metabolism and glucose regulation.

“Our circadian rhythms are often thrown off by things like shift work, jet lag, or sleep loss,” said Clara Peek, Ph.D., senior author of the study and assistant professor of Biochemistry and Molecular Genetics and of Medicine in the Division of Endocrinology, Metabolism and Molecular Medicine at Northwestern. “When that happens, it may be affecting how our muscle clocks function. And if we layer on a poor diet, this combination could make us more susceptible to glucose intolerance and diabetes.”

Circadian rhythms are driven by transcription factor proteins that operate in cells throughout the body, including in skeletal muscle. These internal clocks align bodily processes with the 24-hour light-dark cycle. While previous research has linked circadian rhythm disturbances to metabolic diseases, the specific role of muscle clocks remained unclear—until now.

Peek and her team examined how muscle circadian rhythm disruptions affect metabolic processes, particularly under the stress of a high-fat, high-carb diet. The study focused on mice genetically engineered to lack the BMAL1 gene, a crucial component of the circadian clock that also regulates muscle function and energy metabolism.

When these BMAL1-deficient mice were fed an unhealthy diet, they developed glucose intolerance more rapidly than normal mice, despite similar weight gain. “Eliminating this circadian regulator in the muscle worsened the diabetic-like symptoms,” Peek explained.

Further analysis using RNA sequencing and metabolic profiling revealed that these mice had impaired glucose processing early in glycolysis—the pathway through which cells convert glucose into energy (ATP).

The study also identified a link between BMAL1 and the hypoxia-inducible factor (HIF) pathway, a key system that responds to changes in oxygen and energy availability. During obesity triggered by a poor diet, BMAL1 and HIF appeared to work together to adapt the muscle’s internal clock to metabolic stress. However, when BMAL1 was missing, this adaptive mechanism was disrupted.

To confirm the significance of this interaction, the researchers used a novel mouse model to restore HIF activity in BMAL1-deficient muscles. Remarkably, doing so reversed the glucose intolerance caused by the unhealthy diet.

“When the muscle clock is disrupted, its connection with the HIF pathway breaks down—and that seems to be driving the poor glucose metabolism,” Peek said.

The next phase of the team’s research will investigate whether circadian rhythm is already altered in animal models of diet-induced obesity and whether this disruption directly contributes to insulin resistance and glucose intolerance.

“We already know the clock isn’t functioning normally under obesity conditions,” Peek noted. “Now we want to understand how much that contributes to diabetes development.”

This study highlights the complex relationship between our internal biological clocks and metabolic health—and how restoring those rhythms may offer a new avenue for diabetes prevention or treatment.
